OHIO Part 14
If you
are an unmarried male, and a court has never issued a proper custody order, you
need to seek custody with the court. To do this, you must file a motion for
allocation of parental rights and responsibilities. In Franklin County, you
also need a parenting proceeding affidavit, and a health insurance affidavit.
These are available for download at the Franklin County Clerk of Courts
website, http://www.franklincountyohio.gov/clerk/domesticforms.cfm. The
completed forms are then filed with your county’s Juvenile Court Clerk. In
Franklin County they are located at 373 S. High St. on the 4rth floor.
